A retrospective analysis of patients in the HERA trial was carried out in order better to understand rates of CNS relapse in patients treated with Herceptin.
• The analysis showed that in 3401 patients who were assigned to receive either 1 year of Herceptin or to undergo observation only, 2% of patients in each cohort (37/1703 [Herceptin-treated] and 32/1698 [observation]) experienced CNS relapse as the first disease-free survival (DFS) event.
• The number of patients who died with CNS relapse was higher in the observation group (129/1698) when compared to patients who received Herceptin for 1 year (88/1703).
• Regardless of whether patients were treated with Herceptin, CNS relapse as the first DFS event was more common in patients who had had four or more positive lymph nodes (49%), tumours >2 cm (55%) or hormone receptor-negative tumours (64%).
• Time to CNS relapse was similar in patients treated with Herceptin and those not (1.31 years vs 1.25 years); however, patients not treated with Herceptin typically experienced CNS relapse earlier than those under observation only.

Retrospektivní analýza pacientů ve studii HERA měla za účel detailněji prozkoumat riziko relapsu v CNS u pacientů léčených Herceptinem.
• Analýza 3401 pacientů, kteří podstoupili roční léčbu Herceptinem nebo byli vybráni pouze do kontrolního ramene, ukázala, že se u 2% v každé skupině (37/1703 [Herceptin] a 32/1698 [kontrolní skupina] vyskytl relaps v CNS jako první událost v přežití bez příznaků nemoci (DFS).

Introduce an ultra-stiff guide wire with profile of 0.89 mm (0.035 inch) through the stenosis.
Insert the scope, localize the stricture, and externally mark the ends of the stenosis.
Withdraw the scope and use an appropriate length of the stent for the length of the stenosis. Information about the nominal (relaxed) length of the stent model is presented on the packaging label.
Compress the stent into the delivery system immediately prior to insertion. The olive forms a firm cone preventing damage of the tracheal or bronchial wall during introduction of the delivery system. Carefully advance the delivery system over the guide wire into desired place.
Prior to releasing the stent from the delivery system, position the stent as follows: One third of compressed stent inside the delivery system should extend beyond the lower margin of the stenosis to allow for stent shortening.
Keeping the pusher in a stable position, slowly pull back the sheath until the stent is fully released from the sheath. In most cases the stent will continue to dilate gradually to reach its full diameter over 48 hours.
Check position of the radiopaque markers with the position of the external markers.

Zaveďte ultra tuhý vodicí drát o průměru 0.89 mm (0.035 inch) skrze stenózu.
Zaveďte bronchoskop, lokalizujte strikturu a zevně označte konce stenózy.
Vyjměte bronchoskop a použijte vhodnou délku stentu odpovídající délce stenózy. Informace o nominální (relaxované) délce modelu stentu je uvedena na štítku.
Zkomprimujte stent do zaváděcího systému bezprostředně před implantací. Olivka zaváděcího systému tvoří kužel, který chrání tracheální či bronchiální stěnu před poškozením během vsouvání zaváděcího systému. Opatrně posunujte zaváděcí systém se zkomprimovaným stentem po vodiči na požadované místo.
Před uvolněním stentu ze zaváděcího systému umístěte stent následovně: Z důvodu zkracování stentu by 1/3 komprimovaného stentu v zaváděcím systému měla přesahovat distální okraj stenózy.
Držte pusher ve stabilní pozici a pomalu stahujte kryt zaváděcího systému směrem k sobě, dokud stent není kompletně uvolněn z krytu zaváděcího systému. Ve většině případů se stent dilatuje na nominální průměr postupně, k jeho úplnému rozvinutí dojde do 48 hodin.
Ověřte pozici rentgenokontrastních značek stentu s pozicí externích klipů/značek.
