In the early 2000s, aortic valve surgery was approaching a stalemate, as only two types of primary prosthetic aortic valves—mechanical and biological-were available. In addition, many patients were deemed inoperable with conventional surgery by the attending car-diologist or practitioner because of comorbidities or advanced age per se [1 ].
The advent of transcatheter aortic valve implantation (TAVI), which does not require cardiopulmonary bypass (CPB), has pro¬duced a real revolution in the management of valvular heart disease, providing new hope and an alternative treatment option to patients who were previously considered too high risk for con¬ventional surgery [2].
The introduction of the TAVI technology has gained widespread enthusiasm, such that nowadays, several cardiologists expect to perform TAVI in 'every patient with aortic valve disease', leading to complete abandonment of the surgical approach. In a more col¬laborative environment, TAVI has brought cardiology and cardiac surgery closer together, and therapeutic options are tapered to the needs of the single patients. Therefore, physicians can carefully consider the strengths and weaknesses of each management strat¬egy so as to identify the most appropriate treatment option for their individual patients.

В начале 2000-х годов операция на клапане аорты считалась безальтернативной, поскольку существовало только два варианта протезов аортальных клапанов - механический и биологический. К тому же, многие пациенты после посещения кардиолога или терапевта считались неоперабельными в условиях обычной операции, в связи с наличием сопутствующих заболеваний, либо в связи с преклонным возрастом, как таковым [1].
С введением транскатетерной имплантации аортального клапана (ТИАК), которая не требует подключения к искусственному кровообращению (АИК), произошла настоящая революция в тактике лечения порока клапана сердца, даря пациентам надежду на лечение альтернативным способом, тогда как раньше они попадали в список тех, кто подвергается высокому риску в процессе выполнения обычной операции [2].
Технология ТИАК завоевала такое широкое признание, что в настоящее время группа кардиологов рассчитывает на применение ТИАК "к каждому пациенту с пороком клапана сердца", что в свою очередь приведет к полной отмене хирургического вмешательства. В более благоприятных условиях для сотрудничества, ТИАК сблизил кардиологию и кардиохирургию, что позволило сопоставлять возможные методы лечения с индивидуальными потребностями пациентов. Поэтому врачи могут тщательно обдумать слабые и сильные стороны каждой стр¬атегии лечения, в целях определения индивидуальных подходов к каждому из пациентов.

My English is fluent and I have extensive experience of translating legal and medical documents.
For three yars I have been working as a freelancer with Ukrainian translation agencies.
I read different (medical, legal, psychological, general) topics in Enlish regularly to feel the language.
I had a job at private company and was directly involved in the project of establishing a joint venture with foreign investments. All the legal documents including the Statute were translated by me for our investors and all the company registration documents of our European partners were translated into Russian and Ukrainian to legalize the joint venture. My translation got a notary certification.
Beeing a manager of foreign economic activity I also worked closely with commercial contracts, accounting and banking documents translation.
