@Emmanuella even with CNAS deciphered, we are still none the wiser.

We still have to guess what kind of "certificat" is this form: who issued it? meant for what purpose? supposed to "certify" what exactly? who exactly is the person concerned by "En qualité/بصفته: ACTIF" - the one signing the certificate? the person who needs the certificate? in which way this person (or entity?) is supposed to be "En qualité d'actif"?

A textbook example of how NOT to ask a question.

Yes, we could try to find a website with a collection of all forms related to CNAS and IF found wade through all of them for instances of "En qualité/بصفته: ACTIF" or just "En qualité/بصفته:". That would probably yield enough clues to get a reliable answer.
